Camp Forms for Space Voyage

     Both waivers should be printed, completed, signed, scanned and returned to Space Voyage. Please upload each using the Parent Portal. When scanning, create a PDF for each form, and name it with your child's name and week attending. For example: sally-smith-medical-form-week-1.pdf, and sally-smith-liability-waiver-week-1.pdf.  Please save your scans as PDfs. Thank you very much.
     No registration is complete until both of these forms are on file.  Forms should be submitted at least four weeks prior to the start of your camp week. If you need to mail them, please include a note for our registrar.  If you need help, contact us. Thank you.
